  2. We'll help connect you with the right team.

    Our team will help determine whether Community Networking may fit, what funding pathways apply, and what information we'll need to move forward.

  3. Assessment and planning.

    If the service is a good fit, we'll work with the person, family, and care team on assessment, intake documents, and a plan that reflects the person's goals.

  4. Begin support.

    Once everything is in place, services begin. Plans are reviewed regularly so support can grow with the person.

Is this a center-based program?

No. Community Networking happens in real community locations — libraries, classes, volunteer sites, recreation centers — not at a single program building.

How does it differ from Day Supports?

Day Supports may happen in licensed day program settings; Community Networking is structured around community-integrated activities.

Can the schedule change week to week?

Yes. Schedules are individualized and flex with the person's interests, opportunities, and goals.
